2. Download, complete and cut and paste a recent, clear photo [no larger than 4x6] of yourself into
the talent registration form.
3. Email your completed form to: registration@bamcastingla.com
Register by U.S. Mail:

2. Download, print out and complete the talent registration form.
3. Attach a recent, clear [no larger than 4x6] photo of yourself and mail to:

PARENT CONSENT FORM
A parent consent form is required for all children 17 and younger. Please download and sign the consent and include: (1) A VERY CLEAR COPY OF THE CHILD'S BIRTH CERTIFICATE, (2) A COPY OF THE CHILD'S SOCIAL SECURITY CARD, (3) AND A COPY of a current report card, doctor/hospital record, and/or a copy of your child's school I.D. with his/her picture on it. Fax the signed consent form and these two other documents to 318-698-1312.
NOTE: Due to the Louisiana Labor Laws, all participating children must submit a completed parent consent form and corresponding documents to the casting department in order to be a part of the film!

LOUISIANA DECLARATION OF RESIDENCY FORM -
IF YOU ARE AN ADULT, ALONG WITH YOUR COMPLETED LOUISIANA DECLARATION OF RESIDENCY, YOU NEED TO SEND US A COPY OF YOUR DRIVER'S LICENSE (WITH THE EXPIRATION DATE ON IT), A COPY OF YOUR SOCIAL SECURITY CARD, AND A COPY OF YOUR PASSPORT (IF YOU HAVE A PASSPORT).
IF YOU ARE A MINOR, ALONG WITH YOUR COMPLETED LOUISIANA OF RESIDENCY FORM, YOU NEED TO SEND
(1) A VERY CLEAR COPY OF THE CHILD'S BIRTH CERTIFICATE, (2) A COPY OF THE CHILD'S SOCIAL SECURITY CARD, (3) AND A COPY of a current report card, doctor/hospital record, and/or a copy of your child's school I.D. with his/her picture on it.
